Technically speaking, adding the hreflang sentence will not help your SERPs. Because it's redundant.
Also, I'd advise you to not add it. The reason is simple, the more lines of code, the slower the page is. And there is a common sense that google does not like redundant code.
Think it by yourself, in which way will it help you, that knowing that an english site, having a signal expressing that their language is english? Its like your mom would have a sign with ''I am a woman''

Just redirect 301 the m.xyz domain to the responsive site. That should be all.
Remember that it might impact your SERPs and it might take some time to be completely updated in Google index. Always update the sitemap.xml without the old domain.

Is well known that Search console usualy has those error, that displays information that is not accurate.
Have you tried to search that site? Using site:yoursite.com? There you will confirm how many pages are indexed.

It's not necessary, because you are pointing the same URL, there is no actual canonicalization.
Personaly I believe that it will not do any harm, but its redundant. And it's not advisable to have redundant code.

Page titles are only one in the whole ranking factors. If you have optimized your pages for those keywords, it would be advisable to optimize and work on the other ranking factors.

Also, have you tried the Moz's on-Page grader? Check your pages and your competitors pages for those keywords, pretty valuable information will come out.

Just as Eric said, it's like a problem triggered by the wordpress CMS.
I know 3 different ways to solve this problem:
1- Using a plugin, that creates a redict 301.
2- Creating a redirection pattern in the .htaccess file. Here more info:
301 redirect (www.domain.com/index to www.domain.com) - Q&A Moz.com
3- In the case that redirects cannot be done, use rel=canonical in those pages making the problem.
I believe that this solution would need some coding creativity, to create a code that detects the URL non desired and put the right URL in the rel=canonical tag.

I'd like to state 2 ideas:
1- On one hand when it comes to the usability and making the more user friendy the site, links in the footer will be a waste of time.
2- On the other hand, it's commonly used by companies. I believe that they feel those links as "good practice" in a profesional web site.
A third point would be in a SEO perspective. In my opinion, they have no value. The internal linking should come in other ways, more like a "spider net".

Instead of using trafficTravis. I do recommend (when just tracking serps) using Moz, or some private trackers, like serpbook.com or proranktracker.com).

Does it represent a problem for you, having those redirects in the htaccess?
Personaly, and in a SEO perspective, I will never delete the 301s redirects done in any site.
The fact that those pages havent recieved traffic doesn't mean that the old URL might have any links or authority that is now redirected to the actual URL.
