A very late answer to the question, but you want to put in the 301 (permanent) redirects as soon as you know of a problem. http://www.seomoz.org/blog/duplicate-content-in-a-post-panda-world has some great help regarding duplicate content.

Your SEO have the effect on whole website not only one page :), you make the site optimisation not a page only, this is not a good strategy(one page only but that can affect ).

Page that has no link is being crawled
There are numerous ways for google to find your URL even if it doesnt have a regular link to it - security by obscurity isnt very effective, if you truly dont want anything to be crawled hide it behind a login. I'm not sure how these pages are being created obviously but I wouldnt redirect - instead remove them from index using meta tag or robots.txt. Or just delete them

Can you optimize for 2 keywords per URL?
Only if the 2 keywords are very close together, like 'widget' and 'widgets'. Otherwise, 1 keyword per page works best, or else you'll confuse the googlebot as to what your page is really about and what it should rank for.
