What do i do when all pages are grade A?
I think what I've done wrong in the past is to go to sites in my field and get links from them. I should go to sites outside my field that relates to my specific page or blog, this way it's relevant, not trying to steal their traffic...

Anchor Text Percentage
Right, I understand all of that, but it doesn't really answer the question. My question is regarding anchor text that INCLUDES the keyword vs anchor text that is ONLY the keyword. So is it OK to have a high percentage of anchor text that includes the keyword (based on numbers in OSE's anchor text "terms" instead of "phrases")?

Www/nonwww .co.uk/.com
Just checked with the Rank Tracker and the .com domains don't appear to be ranking for any of my keywords (including domain name). Will I notice a bump in SERPs/traffic once I 301 the domain .com -> .co.uk or has google already figured that out behind the scenes?

How to proceed on this 404 situation
Fabio, If you are taking the time to redirect to a 404 page, I would suggest using a 301 to a page that is active. Instead of wasting the link juice on a 404 page, you can pass it on to something that is active and useful to your visitor (even a "help" page) The key here is to find a new page that is at least somewhat connected to what was on the dead link's URL.

What is the best SEO-friendly on-site user reviews SAAS provider?
I really like Bazaar Voice for their on-site review integrations. They spoke at GR2L during SXSW last year and I was very impressed with their offering.

SEO value of "in the news" links on home page?
I have a feeling that for most it's just a way to try and build credibility by showing 3rd-party endorsement. I think there's a chance is does provide some amount of SEO benefit as far as increasing indexing frequency, as it may be the only section of the homepage that changes with regularity. That said, I think a blog (with a list of most recent posts on the homepage) serves this purpose better. I think unless you're a large organization that get's a lot of 3rd-party media coverage, having a news section can be actually be a negative because: If most visitors are like me, they use the date of the last news post as a judgement of the site or company. Ongoing, it can be difficult to generate updates for the news section. Linking away from your homepage to another site = lost traffic and lost link juice (if you're into that). My $0.02.

Google Places Missing Listings
Without knowing exactly what is happening, you may want to create a post on the Google Maps Help forum which they have in place for exactly these issues. http://www.google.com/support/forum/p/maps?hl=en Are you doing anything with the keywords or anything like that? I have had entries get declined, but they usually still appear on the account. Posting to that forum is your best bet as those folks are sharp and Google will respond and they can look in the back end.

Where to syndicate?
Have you tried PRLeap, PRWeb, and Vocus? I've seen tons of articles get picked up by Google news and displayed at the top of the SERPS without too much work using PRWeb, and Vocus can help you find journalists responsible for syndicating content in all kinds of big name channels. You can also try googling creatively with industry-relevant keywords and advanced search operators to find sites that Google itself currently sees as authoritative, which are also publishing guest articles, then try submitting your articles to them. I would also consider becoming a syndicator of content yourself. Google has guidelines to follow to get yourself recognized as a source for Google news (you have to apply), and it may behoove you to set yourself up to syndicate other people's content (and your own). It would give you tons of control, the opportunity to crowd-source content on your own site, etc.

Crawl complete, but nothing changed?
Here's what it says: Last Crawl Completed: 2011-03-30 12:17:51 -0700 New Crawl Starts: 04/04/2011 Unfortunately, the errors that it mentioned were fixed several days before the crawl started. If it happens after the next crawl, I'll be worried. So, I'll just wait until then.

Google seems upset that I took their advice. [Titles and alt tags for images.]
It varies depending on the work I am doing at the moment but generally 10-20 images maybe?Never more than 20 I wouldn't think, except on rare occasions (big premiere, etc.) ... if you look in my profile under "my portfolio site" you will see a link to the site.
