One client, one b2b site and one b2c site - linking relationship
Search engines understand footer links and therefore the total value of a footer link is greatly diminished. Per a Bing rep, if a footer link exists and is relevant then the value is 1 link from that site, not 10,000. So I don't believe you will get as big of a bump as you might believe. However, I do believe that cross linking your domains is not a bad practice. We host our blog on a separate site and link back and forth. It only seems to help us and not hurt us. But you do run a risk of looking manipulative and therefore being penalized. I would suggest testing one link in the footer. See if it gives you a bump or gets penalized. If you get a bump, keep it. If you get dinged. Get rid of it.

Do keywords ride solo or do they have sidecars?
Just to add a bit to what Joe shared, your "fruit salad" page will surely appear in the organic results for a "cheap fruit salad search". The difference is you are ranking as #1 for "fruit salad" where you may rank well for "cheap fruit salad" or you may be the 10,000th result. There are many factors involved including if the word "cheap" is used somewhere on the page, your competition, etc. In a perfect world your site will have one quality page dedicated to each key phrases you wish to target.

Explain Competitive Link Analysis
The ratio of followed vs nofollowed links on a web page is not a ranking factor. You do want to keep the total number of links offered on a page down to the minimum number required to provide the best user experience. Repetitive links and unused links should be removed. Your page rank flows through your followed links.

What's the impact of # in the main domain page?
I have spent a huge amount of time lately reading a lot of SEO information and have never seen that suggestion made. The closest would be Matt Cutts sharing how he slightly altered his page title from his H1 tag to capture an additional version of a key word.

Will changing domain registration details affect ranking?
In the long run, changing the registration from a "person" to a "company" will give you added credibility. Especially if your trying to utilize the search engines places/local listings. They will send you a postcard to your company address. Once you validate the postcard you'll be considered a real player. Keep in mind, that all goes out the door if you have a brandable personal name like Eddie Van Halen.

Any ideas for capturing keywords that your client rejects because they aren't politically correct?
I remember talking with an SEO copywriter who gave an example of a loan company that wasn't allowed to use "bad credit" as part of their advertising, yet it still drew a lot of keywords. The copywriter would write text like "Have other companies told you that you have bad credit?" I also like the idea about writing articles. You could do something about the history of the term, or myths associated with that term. Another idea might be any quotes from patients or families that could include "I didn't want to go to a nursing home when I got old, I'm so glad my family found Blue Hair Acres for me instead." Remember you can try to work your phrase in across two sentences. "So and so has a degree in nursing. Home-like care is a goal" (cheesy, but it shows what I'm talking about).

Can "poor" subdomains drop PR of the root domain?
I agree. I have seen posts on SEOmoz and elsewhere throughout the SEO community that others are noticing drops in PR with no impact on traffic. I am not seeing a noticeable drop in traffic so the PR isn't concerning me too much but I am curious. Have you noticed anything like this post Panda and do you think the panda update has any impact on the PR directly (I see how it would with poor content --> harder link building)? Again, I appreciate the feedback!

Advanced Question on Synonym Variation Pages!
The new synonym pages are not ranking anywhere significant for their exact synonym - it is the primary synonym page ranking for them. To me this would be proof that they are seeing the synonym pages as duplicate so you may be better focusing on one page per keyword. Although if these pages are only a week old they could take more time to get picked up. As Ryan mentioned, changes need time to settle in properly, you won't see final results in a week. As you mention in point 3 you are definitely going to decrease the strength of each location if it has to share link juice between 8 similar pages, so you are making it harder for them to rank well.

Pay for Performance SEO
Be careful with anyone who does pay for performance that they are doing white hat seo - look at work they have done for others thru yahoo site explorer and ask for references- you dont want to wake up one day and see you have been penalized by google
