Duplicate Websites
There isn't really anything you can do to impact this. This is not a good strategy for your competitor, and I'm not even sure how much it could potentially be benefiting them. Google will realize the duplicate content and then serve the page that has a higher authority as the result that actually matters. If I do a search for bozeman chevroletand montana chevy I actually see the Montana Chevy website coming up for it's root domain search, but even when I search the exact match Bozeman Chevrolet it doesn't even come up on the first page. This tells me that Google has established the origin of the content and higher authority domain as the Montana Chevy website. In reality that may be true or false, but that's what Google has decided. If you run OSE reports on both domains you'll see why Google has decided the Montana Chevy is of higher importance because of all the links that point to it. I would just let your competitor go about doing this, because I don't believe it actually gives them any real benefit.

Anchor text
Abraco.

Are lots of links from an external site to non-existant pages on my site harmful?
Thanks for this - definitely some food for thought regarding how we handle 404s in general... I am more worried about search engines than humans from this type of thing (we have had no referrals from this dodgy site) so would be interested to see if you still think a 301 is the best way to go since the link text may not be appropriate to our site (and perhaps this would be worse?!).

Image filenames
Having keywords in your image filenames is a good idea, but it doesn't harm it to add some dynamically generated strings at the end if that's necessary for technical reasons. As one example, I maintain a site which has user profile pictures, and by adding a dynamic string to the end of profile pictures it prevents the situation where the user uploads a new profile picture but see's the old one because of their browser cache. Definitely use relevant text in your alt and title attributes, but don't sweat a few extra characters added on a filename if it will help your developers.

An Infrastructure Change for a Large eCommerce Site - Any advice?
1. The lose of equity to those unwanted 'style A' pages - I think a series of careful planned 301s will be the solution. If you redirect the discarded pages you might have a gain in equity. 2. Possible loss of long tail traffic to the individual products which might not be caught by one individual page per style. Actually, with lots more words on a page you might have a gain in long tail traffic. The only way to know is to try it... just saying this because it might not be a loss. More important, you might be moving away from a potential duplicate content problem as these pages might be very similar. 3. Internal link structure will need to be monitored to make sure that we're still highlight the most important pages as well, important. This job is always present.

How would you set up campaigns?
It is certainly a long process, my friend. The best advice anybody can give you is to keep your nose to the grindstone, create incredible content, and figure out creative ways to get links from the most authoritative websites in your niche.

Does a part of a keyword also count?
Google certainly recognizes that the phrases are related, but they most likely distinguish these semantically. You'd probably want to work in phrases like "Pain Management" or "Pain Relief" to optimize for the keyword "Pain". Moreover, though, you shouldn't really be paying attention to keyword density when you're creating your content. In fact, you really shouldn't be thinking very much about Google at all when you're creating your content. The very best content is written for people. Think of your readers. Think of what they would find most interesting/engaging/helpful. Appeal to your readers and write like Google doesn't even exist, and you'll find that you've created the type of content that gets links on its own, and gets shared socially without having to "fake it".
