I've personally had good results with it for local search, but it's main purpose right now is for rich snippets. I like to use schema-creator.org to generate the code for me.

My gut instinct tells me to choose the one utilizing the .com/exact-match/
This coming from a customer perspective. When I search for Diving Equipment I'm going to want the phrase, or something similar, in the url. Of course, both of these match that idea. However, the second one allows me to read it as one piece, and my subconscious is more likely to perceive the page as such.
As for the search engines. Using the latter one will give a more clarified answer to what this page is about without additional context. The only point where I would see it being too complicated is from the length of the url; especially with blog posts.

*That being said, I have no documented proof of such, and have never seen a case study for it. It's just simply my interpretation.

It is duplicate content.
However, that might be the least of your concerns. You are going to have to compete directly with Vimeo, Youtube, IMDB and the companies that produce these movies. That's a lot of high dollar companies to fight with for rankings.

I usually see changes every 2 weeks or so. However, it all depends on your vertical. How many citations do you have compared to your competition?

I think the importance is very slowly diminishing (but will never really go away). However, I believe it is necessary when you are in a competitive vertical with exact matched domains. That way you can perfect the rest of the on-page parameters, and still not come up short because of an exact matched domain.

RE: Duplicate content? Not sure.
All of those occurrences are part of a marketing piece referencing a coffee cup. Due to the structure of the content, you are probably going to have to rewrite that piece. So, that will delete this exact phrase, but create a new one.
I would suggest not using a piece of the content from that article on every page of the website, and instead use a summary piece to describe the coffee cup. Not only will improve your click thrus, but you won't make people read something so depressing so often

Here's my current approach (minus my own little nuances).
- Use http://mergewords.com/ to generate a big list of keywords
- Copy and paste (2000 at a time) into the Google keyword tool
- Make sure it's set to exact, and show related keywords only
- Download the CSV into an Excel Spreadsheet until all keywords added
- Sort by search volume

Outbound Links can be beneficial to pages if they are linking out to authoritative sites. You have to analyze the number of outbound links, as well as the nature and quality of the links. If they have 1 link going out, but it's to something completely unrelated, it was probably bought or an affiliate. If they have 15 links going out, but they are citing additional high quality resources, then go after that link!
