Im sure a lot of us here will recommend the popular CMSes like WordPress, Joomla, Drupal, etc. I think they are all good CMSes. But you ask me, you can use any. The only thing you really need to consider is which CMS has the most support when it comes to hiring a developer or designer to do customization work on your site.

i think the problem here is, if you longer work with that designer, will you be able to find someone who can work on updating your site powered by textpattern? i think most modern CMSes (like Joomla,Drupal,WP) are now built to be SEO friendly. If you use a CMS you never heard of, it will be hard to update the site in the future.

i think the recommended 100 links is to make sure your page is as its maximum effeciency. but in the real world, not all sites can maintain 100 links per page. i think what you can do is choose only those links that gives you the most pageviews / clicks and the rest can be placed on the inside page of that category. this means you only need to have the links that you really, really need the visitors to click on from the page.
for search engines, having a sitemap really helps a lot. you might also need to think of adding related categories on all your category pages to you have a good internal linking structure.

yes facebook account pages and facebook fan pages are indexed. you can see samples here
maybe you are adding a privacy filter that your site wont be indexed by search engines.

we tried to run facebook ads linking to our sales/landing page for several weeks and we concluded facebook ads is not for us. google adwords performed much better and its expensive to get a lead from facebook clicks.
i think it depends on what you are advertising. if you want to increase your facebook page likes, go for it.
also, not everybody are adding correct and complete info on their account profiles, you might be advertising to the wrong people all along.

yes, i recommend to 301 redirect Website B to Website A since it has more backlinks than B. Its easier to move files on your end than to contact those linking to you to change their URL.
i remember SEOmom said they bought SEOmoz.org because SEOmoz.com was taken. Now that they got the .com, they just redirected it to the .org most probably because it already has tons of backlinks than the .com.

Here are my recommended first things to do for a new site
- Write a PR about it and send to PR sites
- Write an several articles about it and send to article sites
- Tell it to your friends on Social Media (Facebook Twitter etc)
- Start an email campaign to your existing email list announcing your new site
- Get bloggers to blog about your site
Make sure the site has no errors and well designed with useful information to encourage repeat visits.

for #3, this is after you have a site. when people starts coming in, you will have new sets of keywords to work on. these keywords helped you get traffic, its best to rank on those new keywords to increase your number of visits.

Here's what i recommend to get you started
- think of what keywords do you think people would type when wanting to learn more about that skin disease
- go to google adwords keywords tool and wordtracker and other keyword research tool to get a list ot popular related keywords
- on your site, add google analytics or any similar website statistics tool to log all keyword searches from other sites that provided the most relevant traffic
- based on those keywords from 1,2,3, get more keywords from competitors that ranks on the same keywords you are targeting.

also you need to host the country targeted domains on the specific country you want to market to. -

- since it is alphabetical, maybe you can only show the popular links, and add a More... link on each letter.
popular links could be
- those with most traffic
- highest revenue
and count in http://www.yellowpipe.com for total link count.

- If you really need all the links, maybe this will work for you:
you can also use ajax to load the links from the outside
one example is http://webdesigner.ph
the content for each page there are loaded from other pages into a single page
you can verify in http://www.yellowpipe.com , it shows the content outside and links are not counted.
in your case, create 1 page for each letter and you can have as many links are you like, it wont be included in the homepage link count since its loaded outside.

maybe you can try using php explode in the canonical url

how about changing the non-logged in version to http and logged in to https ?
this way you dont need to 301 redirect those link juices you are getting from http
