Robots.txt for subdomain
Robots.txt work only for subdomain where it placed. You need to create separate robots.txt for each sub-domain, Drupal allow this. it must be located in the root directory of your subdomain Ex: /public_html/subdomain/ and can be accessed at http://subdomain.root.nl/robots.txt. Add the following lines in the robots.txt file: User-agent: * Disallow: /As alternative way you can use Robots <META> tag on each page, or use redirect to directory root.nl/subdomain and disallow it in main robots.txt. Personally i don't recommend it.

Does a part of a keyword also count?
Google certainly recognizes that the phrases are related, but they most likely distinguish these semantically. You'd probably want to work in phrases like "Pain Management" or "Pain Relief" to optimize for the keyword "Pain". Moreover, though, you shouldn't really be paying attention to keyword density when you're creating your content. In fact, you really shouldn't be thinking very much about Google at all when you're creating your content. The very best content is written for people. Think of your readers. Think of what they would find most interesting/engaging/helpful. Appeal to your readers and write like Google doesn't even exist, and you'll find that you've created the type of content that gets links on its own, and gets shared socially without having to "fake it".

HTTP 404 for 404-page?
That's one where you do want a 404 returned! That should be fine, and kudos to you for having made a custom page.

Status 404-pages
Hello! I am seeing a lot of my clients using a 301 when the page doesn't exists redirecting to a well designed 404 page. But, essentially, the server have to output a 404 HTTP Code and show that fancy 404 page to the user. After all, the HTTPs codes exists to be used, and this is a important one! Hope it helps! =]

Keyword rich footer links negative effect
I don't think there is any hard evidence on them, Rand talks about the footers and manipulation here: http://www.seomoz.org/blog/6-changes-every-seo-should-make-before-the-over-optimization-penalty-hits-whiteboard-friday There is evidence on the fact that if they are linking to a page that has already been linked to higher in the code, they count for nothing. Only the first anchor text links count in google's eyes. So unless you're linking to pages that are not within the nav, it's fairly pointless from an SEO point of view. http://www.seomoz.org/blog/results-of-google-experimentation-only-the-first-anchor-text-counts I think using the Footer for linking to pages already linked to is a waste of time. People use the navigation to find the page they're looking for. Footer anchors should be used for the stuff people expect to see there, i.e. T&Cs, policies, trust signals, awards, contact us, address, Company No. etc.

IP block in Google
yup, i know that problem. too many requests to Google from the same IP. The IPs from which you query Google should frequently change, otherwise you'll get blocked.
