To work on the duplicate problem, i think the best option is the cannonical tag. You will need to include in every page (pages you want to be the official one) inside the head tag the follow:

Than, to block old pages that you don't want to be at google, you can use the robots.txt file. There you will use the disallow function.
I don't know if I understood coorrectely, but I think the follow:
Redirect example.com to www.example.com RewriteEngine On RewriteCond %{HTTP_HOST} ^example.com RewriteRule (.*) http://www.example.com/$1 [R=301,L]
Redirect especific page redirect 301 /old.html http://www.example.com/new.html
I holpe this is what you are looking for...
I don't know any software to do this job. But did you tried the tools from SEO MOZ? The opensite explorer give you a lot of information about your website and the competitors. And you can use credits to go for a more detailed information.

Man, i think i have bad news for you: your site could be penalized. Usually when the site drop down for several keywords, it was penalized by google. Please take a look at: http://www.seomoz.org/blog/how-to-handle-a-google-penalty-and-an-example-from-the-field-of-real-estate
I also think is impossible to remove the link (you need to contact the website). I have a problem from a SEO directorie with a bad anchor text. It was not possible to cancel this.

If all the links for your website is followed, this should also be a problem. Its not so normal, and in my opinion this should affect google.
But some social medias have also followed links. In my website i have a followed link from formspring. I also read in the Q&A that Delicious make followed links for websites with more users sharing it.
Another tip: did you tried the SEO directories? You can find a list here: http://www.seomoz.org/directories. I was new here triyng to find links, and I was afraid from the directiories. I tried and it was perfect. For sites that have not so much links, its a perfect solution.

Than you will need to make a 301 redirection to solve the problem. You will need to edit your .htaccess file. You will find it at you public_html fold. If it doesnt exist, create one.
Inside this file you in write the follow:
RewriteEngine On RewriteCond %{HTTP_HOST} ^web-writer-articles.co.uk RewriteRule (.*) http://www.web-writer-articles.co.uk/$1 [R=301,L]
Problem solved and google will not more complain.

This one allow you to create patterns for your pages. You can change all pages in only one time, filling all meta fields in an easy way.

I think nowadays rel=nofollow there is no meaning. With the changes in google, also the link juice is not been affect by it. You can check some info in this old post: http://www.seomoz.org/q/do-you-use-nofollow-and-rel-nofollow
But in SEO's live everything is gray, and not black in white. Its better for you using 100 links per page and go with more security.
