Proper 301 in Place but Old Site Still Indexed In Google
How big is the site in question? How many pages are there to de-index? What does Google Webmaster Tools tell you about the old domain? Does it show pages being removed from the index over time? If you do a site:{old domain} query, can you see that the number of results being returned is gradually decreasing? How have you implemented the redirects? Have you submitted a change of address request in Webmaster Tools? On the new website, have you submitted a sitemap fom the old website as well as the new one? What does the backlink profile on the old domain look like? Can you start to get authoritative links to the old site updated? What about any embedded internal links in your content - have they also been updated? More guidance from Google here: Webmasters Tools Help: Move a site with URL changes

What Happens If a Hreflang Sitemap Doesn't Include Every Language for Missing Translated Pages?
Hi Kyle, I would probably only include the URLs that have been translated in the hreflang sitemap, unless the English content that hasn't been translated can be assigned to one territory / language, e.g. en_us or en_gb, etc. If the pages that haven't been translated are otherwise linked-to throughout the site and are present in a regular sitemap, they will be found. I am not 100% sure that leaving them out of the hreflang sitemap is the way to go, so I will leave this question open for other people to reply as well. However, I'd say that if you wanted English language content to serve internationally because it has not been translated, it would be a mistake to target it to one English-language speaking area via hreflang.

Best Blog Post Length to get Links
All the points made are great and I'd just add one more thing...look at the average length of the blog/website that the post is going on. Like EGOL said, on Moz, most posts are around 1000 words so that's what you should aim for but for another blog, they may want something longer or shorter. Match the content and length to the blog it is going on, that way you are adding value to the blog and not just blogging for links.

Links Removed from Site but not Google Webmaster Tools
Kyle, if you have done a domain level disavow, there's no need to do another URL by URL. We are seeing disavow take anywhere from 2 weeks to 2 months to take effect. Wish I could offer better news, but it looks like you're in a hurry-up-and-wait mode.

Pop Up Advertisement - Bad for SEO?
Good point on the a/b testing, perhaps i move forward with that first to help build some data to support my case. I agree to the end user pop-ups are terrible but i am still intrigued if any data supports it is terrible for SEO?

Shared Hosting - Bad for SEO? (exp. Godaddy)
one thing to keep in mind is server down time and server speed. A lot of websites can be affected by having a slow host or the website being deindexed for significant downtime. I have a client right now that is on network solutions and had his site deindexed because it was down for 5 days because of a unix environment issue. The website is still taking 8-12 seconds to load and is killing his traffic. We are in the process of moving it now ;).

Clients Slow to Publish Content
I prefer to stay away from ranks unless its a document that says we did "x" and as a result our ranks improved by "z positions" and "traffic grew by o%" and "sales or leads increased by n%." Think of it as a 1, 2, 3 or 4 process flow chart with the money on the end result. I include annotations on algorithmic updates, new/updated content uploads and outbound marketing activities. Basically anything that can create a social or normal link to your website. If the activity creates links then it should be annotated on the graph. This includes web development changes to your website. It's difficult to communicate results when one tries to show what their plan was or is. I stay away from things like that because the purpose of that graph is to communicate results. The line graph should be simple, visual and full of impact. See if you can get one or two high quality articles out the gate. If you can measure the process like I mentioned above then show the resulting positional movement on the SERP and conversions. See if you can add $dollar figures to your numbers. Try to educate the client and show that 'x articles' can result in 'y leads' or 'z dollars.' Then probably put together a document that says if we produce this many articles then this is how much we can get in sales. Make the business case. It's always a constant battle. Sometimes I like to do the reverse, especially when the client is so slow moving that I'm either losing faith in myself or getting downright frustrated. For these type of clients, I put together a business case that shows how much money they are losing and have lost since I've started working for them because they haven't moved fast enough. Simply calculate the number of articles that produce x amount in sales for a single month. Multiply it by 12 or how many months you've been employed by them or say something like - "You've been leaving xxxxxxx dollars on the table every year because of x, y and z." Try this tactic only if your numbers and calculations are solid and if your numbers are substantial and you will usually get a good reaction out of them. I hope this helps.

Disavow Link Tool from Bing Webmaster Tools
It's cool, but it feels like Bing trying to grab a little attention. I doubt they're at a point where they can process link data for a Penguin-style smackdown the way Google can. I do wish Google would do the same. In spite of the fact that it might lead to some abuse. Right now I could basically run a blackmail scheme against legitimate webmasters by pointing crappy links at their sites, then telling them to pay me $500 to take them down.

Conversion Rate Optimization (CRO): Best Approach/Plan Documentation
If this is something you want to present to your client, it's probably best to explain the procedures of A/B testing. "Hi [customer], You are finally getting traffic to your site, however we want to run some A/B testings to maximize conversions on your website. We plan on running these tests and will only have results after 30 days (or more). " The site I mentioned above will give you reports that you will be able to present to your customer after experimenting: http://visualwebsiteoptimizer.com/features.php#revenue-tracking
