Hi there, thanks for your question! Rel=Publisher is for brands, while Rel=Authorship is for people. As the code snippet fork Rel=Publisher should only be placed not the most important page of your site, placing it on the home page is a good choice. As the markup tool is verifying your Publisher setup, I'm assuming you have correctly placed a link on your Google+ brand page back to your site's homepage. Well done.
Yes, the Rel=Authorship code snippet should be placed on all article pages and blog posts --- that have a byline for a particular author, that is. You then need to establish a link from that particular author's individual Google+ profile back to your site. There are a few ways to go about this. Hi there, thanks for your question! To answer your question directly, Moz would be crawling your old URLs because it is able to find them using internal links and/or your sitemap.
You can verify that Moz is crawling your old URLs -- and even see where they are finding them -- using Moz Analytics. To do this, log into into your Moz Analytics dashboard and go to the Crawl Diagnostics report, located under Search in the LH nav. The Crawl Diagnostics report will list the URLs of pages Moz has identified as duplicate content on the High Priorities tab, and how many internal and external links each of those pages has. To see the URL Moz crawled to find a specific page listed here, download the CSV file from this tab and look at the column labeled "Referrer".

Thanks for the clarification, Wouter. Given that information, I would go with info@ourcompany.ni. As it is gmail-hosted (and not forwarded), and the only email address that the public interacts with, it should give you the greatest advantages described in Rand's video -- and any that Google+ may roll out in the future. I just did a nonpersonalized search in Google-US for Volunteer Veterinary Hospital in Knoxville, and you client came up as the first organic result in the SERPs. My hunch is that you are not ranking for the search terms you are tracking in the tools you mentioned, and you are not tracking the search terms that are actually driving organic search traffic to your client's site.
In order to tell for sure, though, please provide us with more information about the tools you are using, and what, exactly, they are showing you that has led you to believe that your client's site has no presence in Google's search results.
Finally, if your client's site has been verified in Google Search Console, (formerly known as Googlewebmaster Tools), you can see how often the site is showing up in the SERPs, and for what search queries, in the Search Analytics Report.
