You need to contact the sites directly or tell the SEO Company to do it. Its much harder undoing low quality links than building them!!

Sounds like it could get messy to me. As google always crawls from the US they will be redirected to the .us site, while the rest of the world will not... could be seen as cloaking if not done properly.
I'd always go with subfolders and have .com for the US then .com/uk/ etc. see Apple as a good example.

Its not just from backlinks. I've got blogs where the homepage is PR1 but posts are PR3, due to social signals and shares of the post. This seems to be happening more recently.

You could use Xenu or Screaming Frog to pull out all their page meta titles (assuming they are optimized for keywords).
To find the keywords they are really going for, use something like MajesticSEOs frash crawl index to see the top pages they are building links to... the anchor text used will be the give away.

You are producing duplicate content on the monthly pages e.g.
http://www.nuxeo.com/en/about/events/apr2012training-boston
http://www.nuxeo.com/en/about/events/jan2012training-boston
Cut and paste a chunk of the page text into the Google search box, you will then see all the duplicates being indexed by Google.
