I would release the guide as soon as it's approved so can get indexed. Also, write verbiage on expected eta. Often times, these eta's are not made, so be broad: expected release q2 2018 or whatever. Good idea to create a sign-up form to collect emails so you can send off updates & firm release dates. Good luck!

Whenever I disavow a link, I also will ask the webmaster of the incoming link to remove it and document the correspondence in a Google Doc. This documentation can be extremely valuable in a reconsider request if Google penalize your site in the future that you made an effort in following their guidelines before the penalization.

With all things being equal, an older domain is typically better for SEO reasons. However, if you are doing your redirects correctly to the new domain, should be no issue.

No--best practice is to describe the page as best as possible and this typically consist of using keywords. If you try to stick many kw's in the title tage, it will look spammy and should be avoided (the two examples you gave, IMHO, are perfect).
Is using your company name in the title is acceptable? Yes it is--however it should be secondary and creating a good compelling title (that closely matches searcher's query and page content) should be top priority.

RE: SERPS appearing and vanishing every 15 days
Very strange. When checking the organic position, are you logged out of your google account? If the "true" organic position is on page 5 (or whatever), and when you are logged in your Google account you are on the first page could cause the position fluctuations but not the timing frequency (unles you do this every 15 days). That is my guess.

RE: PDF or HTML Page?
Pdf's are great--but html is better. For a few reasons: Usability (html pages look better in a browser), lack of navigation (however, we can have hyperlinks in pdf's), PDF Download/View is an extra step (ways around this, but for the most part) and etc.
Matt Cutts sums it up pretty well during an interview w/Eric Enge (from 2010):
"We absolutely do process PDF files. I am not going to talk about whether links in PDF files pass PageRank. But, a good way to think about PDFs is that they are kind of like Flash in that they aren't a file format that's inherent and native to the web, but they can be very useful. In the same way that we try to find useful content within a Flash file, we try to find the useful content within a PDF file. At the same time, users don't always like being sent to a PDF. If you can make your content in a Web-Native format, such as pure HTML, that's often a little more useful to users than just a pure PDF file."
**"**People can certainly use that if they want to, but typically I think of PDF files as the last thing that people encounter, and users find it to be a little more work to open them. People need to be mindful of how that can affect the user experience."
Certainly, you can have both and there are many best-practices you need to implement before doing this.

I don't believe the two events are related. It's more related to Panda 4.0 and their use of thin content and doorway pages.
According to an excellent article on wordstream by Larry Kim: "When you search for specific products and see an organic search result, chances are, it's leading you to a doorway page – an internal search results page on eBay with very little content..."

The Googlebot will crawl this information. However, Google may elect not to index it or discount this content in its rankings.

The links may have just been removed. Also, there rankings may/may not been influence by these tactics (they been around since 1998). As you know, many factors matter in organic ranking. You can make a report of webspam at https://www.google.com/webmasters/tools/spamreport.

To the best of my knowledge Google will read it pretty regularly (every day/several days). However, Google takes several weeks until the disavow request is processed

Yes, match as closely as possible. You can also preview if the redirects works good by changing your host file and opening google and query site:yourdomain.com and clicking on each page to see if it redirects correctly. Good luck!

If the blog post is under 2000 words and have a singular focus, I would just have it on one page (better for search, printing & etc.). If it split, the objective may be for advertising to increase pageviews.
