Having a testimonials page on your website is a great idea. It shows the clients going to your website that you are a good provider. I would think you could just place "Review from Google+" or "Review from Yelp" at the bottom of a comment.

We don't place reviews from other websites. The testimonials we have posted came from emails, letters, etc. - However, I would think that if you linked the word "Google+" from "Review from Google+" with a link to that review, it would just be proof that review does exist.

What is the word on Meta Keyword Tags? Are they good to have, or bad?

That may be why they are not going away. When using the disavow tool, you need to find the exact link from that domain where the link is placed, and then disavow that linked page.

The problem is, our content is unique to each page and post. Yet they still have been coming up duplicated. I'll have to try Netrepid's google searching suggestion to double check these.

RE: Traffic from YouTube
From my understanding, you're asking if you should have tracking from youtube to your website, or if Google Analytics will show that traffic based on how much traffic your YT is getting.
If that is the case, then I would suggest tracking them individually as you said. The Google Analytics tracking only tracks how many people go to your youtube, not how many people go to youtube and click your website link.
Therefore, Google Analytics will not show traffic going from youtube to your website, only your traffic to youtube.

RE: Website errors?
In the top right you will see "MOZ PRO" click on that. This will bring you to your home page that will have a listing of your website. Click the "View" button next to it on the right hand side.
This will bring up your dashboard and on the left hand side you will see a menu like this:
Dashboard
Search
Social
Links
Brands & Mentions
Click on the one that says "Search." From there, scroll to the very bottom and on the right hand side there will be a list of the errors you're looking for.

This recently happened to us. Unfortunately, we had to call Google directly in order to get this problem fixed.

You can try to contact the website owner to see if they will remove those links. However, spam websites will never do so. What you can do is disavow the links, not just the domain. Finding the exact links you're listed on and disavowing those is the proper way to clear those links.
Also, using Google Webmaster tools is a great way to find out if you have been penalized for something in regards to your website.

We have been going through our links and weeding out the bad ones. Just like Monica had said below, it is about quality, not quantity. Making sure your links are legitimately related as well can greatly help.
Although we are still out searching for those link opportunities, we have found that one of our biggest competitors seems to be gaining these trustworthy links extremely fast.
Any suggestions as to better ways to get these links would be greatly appreciated. We have slowly been detoxing the links that do not relate to our website listed on other sites. Also, if you are working with an SEO company, keep a careful eye at how they are working on your site as well. We found many of the links we have had to detox are absolutely not related to our site.

RE: Duplicate Content In Webmaster Tools
If you do not edit the URL in WordPress yourself, it will make one for that blog post automatically. If Google webmaster tools is showing the original URL (long version) as a 404 error, it is because WordPress has a shortened version and Google is still reading the elongated one which does not exist.
To fix this, you can redirect the longer URL to the shorter URL with a 301 re-direct - or you can simply go and change the URL of that blog post to the longer version.

If you found the link from that domain that is causing the problem, I would suggest disavowing the exact link using Google Webmaster tools.
Spam links are considered toxic links to your website and can definitely impact your ranking.
