Bernadette said it well. It won't necessarily hurt you, but user experience is very important. It's helpful to see unique information about each page in the meta when a user searches for something; that way, they know to click on your page for their search.
Also, it's a great best practice to give every page a unique meta description and title no matter what.
If it's the bounce rate and not the website or landing page visits, then it's your content. You need to rewrite or reword your content so it's more engaging and interesting. Structure it so the most important, interesting information is in the first paragraph, but leaving them wanting more. You should also be making sure that each one of your Facebook posts accurately describes what's on the page. If you feel like you have to jazz up the description on Facebook, chances are the copy itself won't keep anyone's attention.
We had a plastic surgeon client who had the exact same issues. What we did is we brainstormed the results. How the person would feel, what the person wanted to accomplish, etc. What we figured out worked best was to appeal to those end results and positive desires without trying to manipulate someone's feelings. For example, you could use the confidence angle, like "Walk into a room with confidence" and whatnot. You'll probably notice that there's always an underlying cause for people to wish to get cosmetic surgery, whether it's health reasons or a personal goal, or self confidence, so use that knowledge to better reach the people who are more likely to use your services. It is very common for keyword rankings to fluctuate almost constantly, for a few reasons: search engines are constantly updating their ranking algorithms; rankings are based on user behavior-based signals like how often a link is clicked, bounce rate, etc., and those things are never constant; depending on your location and previous search results and actions, search engines will change the results you see personally; and on and on.
It's better to look at trends in your aggregate rankings rather than individual rankings, and also to focus on things like providing great content and building an authoritative linking profile than to focus on individual rankings on a day to day basis.
Overall, I do think you will see more stability when search engines have more data about how users interact with your site, but I know that the fluctuations in rankings will never stop. They will likely become less drastic, though.

A title tag is limited to only 70 characters so you want to use this small space wisely! (Anything over 70 characters will not help you, only hurt you and confuse search engines) Given this small amount of space, you do not have room to put your company name in there. You may have room for your name in the Meta Description, but never the title tag.
In the title tag you want to enter keywords and location modifiers. Let's do an example. Let's pretend you are creating a page for plumbing repair service in Tampa, Florida. You should use the title tag:
Plumbing Repair in Tampa, FL | Florida Plumbing Service & Repair

I don't see a reason why Google would penalize you for putting the signup box midway through your blog content unless it creates a negative user experience; Google does track signals like bounce rate and time spent on page, and if the box placement ends up causing people to leave your page sooner or leave your page before taking another action, you may see your listing ranking decrease. However, if placing the box higher on the page results in more conversions and does not detract from user experience, you shouldn't be penalized for it.
At our online marketing agency, we stress the following phrase: track, test, tweak, repeat. Why? Because it can be frustrating to try different SEO strategies without knowing the outcome. Like others have said, patience is key for success in SEO. Want to a new SEO practice? track them and test them... and if they don't provide the results you were hoping for, tweak them and start the process over again. There could be several different issues you've run into here:
One issue could be that there is a site coding error. I would refer to your team of webmasters for this and have them double check all URLs and coding structures to see if they can determine the issue themselves. It is possible that Google is having a hard time reading, crawling and indexing your pages.
Go into your Google Search Console and check your inbox/messages! Whenever Google finds an issue with your site (and it looks like in your case, it has) you will receive an email in your Google Search Console inbox.
You should still have specific results for Google Referrals, this data should not have randomly turned into organic.

When writing blogs for our clients I make the photo link to "none" UNLESS it is a brand logo or something of the like that can link to the brand/manufacturer's website. I find that this could negatively impact the user experience if they click the photo only to realize it brings them to a webpage that only shows the image.
The most important information when adding a photo is the ALT tag. This lets the crawlers know what the image is of.
If I were you, I would ensure all the photos being added have an image ALT tag but do not link to just the image file to ensure positive user experience.

Hmm, that is weird that they are not showing up. I would try submitting the URL into Google's URL submitter: https://www.google.com/webmasters/tools/submit-url
Hopefully by doing this, Google (and Moz) will index the correct title tags so when you use the Moz Page Grader it pulls the most accurate information.

I have to agree with EGOL when they stated, "You are asking if Method A is better than Method B. I am willing to bet that the implementation is more important than the method."
I think either of these methods (button v. form) can work better, but this is dependent on a few things. You will need to consider what the industry is, who the viewers are, and what they are more likely to do. Aesthetics and verbiage will also make a major impact—so it really comes down to multiple factors. For instance, someone who is younger might be more inclined to simply hit a button for ease of use. Making it less clicks will always improve your conversion. But an older person might prefer to fill out a form—but they might also not want to give out their information. My biggest suggestion would be to sit down and ensure you are thinking about your audience, not necessarily what you think would convert the best.

Moz has a great blog post that discusses how many links is too many. I would check that out for advice—while there isn't an exact number of links you should not exceed, many people suggest having fewer than 100 links per page.
If you main navigation as a lot of links I would be worried about the link juice. When your website receives a backlink from another website, hopefully with a high domain authority (YAY!), the link juice is being spread out to ALL the pages being linked from the page they are linking to.
This means if www.cnn.com (who has a domain authority of 96) links to your company's homepage, they would be spreading some of their authority to you through "link juice". But instead of retaining majority of that juice/authority on the homepage (thus increasing the authority on that page), you are going to be spreading fewer and fewer amounts of that authority through all 100+ pages linked from the main navigation.

I agree with the two methods that both you and Gaston have pointed out.
The downside to reversing those links is that the domain authority could drop a bit—which could impact their rankings on the SERPs. If this happens, the client might think you are doing something wrong and causing their rankings to rank when, in theory, you were trying to help get rid of any sketchy links. In my opinion, I’d keep them. They’ll make your work perform better. Disavowing them could yield worse results than what their former SEO provided. If that happens, you're playing defense and blaming.
