We use If By Phone quite extensively and have always had a positive experience with them.
We've tried the affiliate route for lead gen on the CPL model and even though the ratio for leads/deal is high , it did turn out to deliver a positive ROI for the client. You can also try contacting agencies that specialize in this kind of lead gen and buy leads on a CPL model from them ( go only for exclusive leads or you'll be fighting 5-6 other competitors for the business)

One of the verticals we work with is the legal industry (lots of lawyer related sites). In our experience, we have gotten good results with both structures, sites with pages just one level down from the home page and others that have pages 2 or 3 levels down. In both instances the pages in question were ranking quite well and a lot of times at the top of the search results. In Matt Cutt's own words:
I would recommend the first-order things to pay attention to are 1) making great content that will attract links in the first place, and 2) choosing a site architecture that makes your site usable/crawlable for humans and search engines alike.

In our opinion, the change in primary and secondary navigation (combining it) should not impact your overall SEO efforts as the content is still there, (you still have the secondary navigation terms merged under one nav)
If you have really good local ranks for the franchisee pages, then i would suggest rolling out this change in phases. Start by changing the territories that don't have as much activity in terms of search volume. Make the change and observe the rankings over the next 30-60 days. If there is no noticeable impact , then you can make the change across the board. Better to be safe than sorry

Since noone responded , thought we'd chime in. Your assumption would make sense in this case based on all things being equal (Ceteris paribus) haven't used that since University:) . If source A and B are giving you accurate data when it comes to phone calls, we can assume that source C is also the same when it comes to the calls. The only accurate way to test this would be to initiate some sort of call tracking ( even if it is only for 30 days) on that referring source and confirm it for sure (then revert back to the usual #)

Try not just focusing on the exact keyword but also closely related keywords. If the article is well written and picks up good social traction , you might end up ranking for different closely related keywords, not just the main keyword.
Choose your clients carefully Simon especially the ones that don't pay you a lot of money. They are the ones that expect the most amount of work to be done for them and will be on you everyday demanding proof of work. Manage the client's expectations with results from day one. Good, clean SEO takes time and delivers long lasting results. It's all about the education and transparency with the clients.

A quick check of your backlinks IN OSE shows some other questionnable links like submityoursite.org , freeadddirectory.com, addurlshopping.com, txtlinks.com
Did you submit them for non inclusion as well. Perhaps Google is referring to those links not the relevant ones.

There are no standard conversion rates as there are a lot of variables associated with each landing page . (Call to action, conversion elements , industry etc)

If your home page has content that is related to the inner page then there is a possibility. With all the Google updates, super relevant in depth content pages related to keywords have the best chance for ranking.
You should also look at the sites ranking on the first page and analyze all the on page and off page factors that are getting them to rank. Compare these factors with your own site and work on them to make it better.
The site below has social media properties and other sites coming up before it's own listing even for the exact search of the site name. Any ideas why this is happening?
