That's a great question. I would keep the main categories as close as possible to the root domain.

My silo structure would look something like this:

webiste.com/washing-machines - Main category

webiste.com/washing-machines/safety-issues  - Subcategory

webiste.com/washing-machines/reviews - Subcategory

webiste.com/washing-machines/spares -Subcategory

webiste.com/washing-machines/buying - Subcategory

Yes, that's exactly how I would do.  I would use the main category page as an index and link internally from that page to your subcategories with the exact match anchor texts of your targeted keywords.

You still can rank your home page for "appliance repair", and your inner page for "appliance repair Los Angeles". However, I see that you are a local business that provides services only in Los Angeles. In that case, I would only use one page( home page) to rank for "appliance repair Los Angeles" and "appliance repair".  Make sure you have content on the home page about "appliance repair Los Angeles", Meta Tags, and some of your backlinks need to have "appliance repair Los Angeles" as an anchor text. Keep in mind, never repeat the anchor text. You should be able to rank one page for both terms.

It is possible that you have more internal links that are going to this page http://www.bigtrailerrentals.com/flatbed-trailer-rentals/equipment-24 than http://www.bigtrailerrentals.com/flatbed-trailer-rentals/equipment-deckover

Sometimes Google picks one page over another one for no reason. You can redirect the second page to the first one and migrate the content from the second page to the first page and then you should be able to rank your first page for your targeted keywords.

Having a blog on a different server should not affect an SEO, but having a blog on the subdomain will affect your SEO efforts.

I would recommend to build SEO pages that would target each city where your client provides building maintenance. This should help you to improve your Google Ads Score on each page.  You might need to build some links to those pages in order to rank them.

I have used this strategy in the past, but it is still working. Create a clean silo structure, so it will be easy to track in your Google Analytics.

These two pages are targeting the same city in the meta tags. You need to update your meta tags on these pages so they are not duplicate. Also, you can add a unique on-page description to each page, so Google will not treat as a duplicate.
