Best way to permanently remove URLs from the Google index?
I agree with Paul, The Google is re indexing the pages because you have few linking pointing back to these sub domains. The best idea us to restrict Google crawler by using no-index , no-follow tag and remove the instruction available in the robots.txt... This way Google will neither crawl nor follow the activity on the page and it will get permanently remove from Google Index.

15% Drop in Traffic. Anyone have theories about the most recent update?
Google has been lowering the value of low quality links such as non relavent guest blogging, because they dont want people to game the system. If you want to rank in Google you need to give the users what they want (value). Google has not officially announced an update, however they have been making changes, and they make about 500 changes a year. If you go to Googles webmaster forum you can see that many people are experiencing loss in rankings. Why are these results being showed as blocked by robots.txt?
Hi, Your robots.txt file is very .. steroid healthy. It has his own universe Are you 100% sure all of the entries are legit and clean ? First thing I would do is to check Web M;aster Tools for the mobile subdomain. If you don't have it yet, that will be a good place to start - to verify the m subdomain. Once in WeB Master Tools - you can debug this in no time. Cheers.

How does Google determine 'top refeferences'?

Age of a re-directed domain same as age of a static domain?

Keep older blog content indexed or no?
Like what James said, I wouldn't de-index them. Perhaps consider putting a call to action to get those visitors to micro-convert to an email list or put some sort of offer on them? Just a few ideas...

We're indexed in Google News, any tips or suggestions for getting traffic from news?
I imagine this would have to do with social sharing usage. If your posts are getting a lot of attention via Twitter, Google Plus, Facebook, etc within the first few hours, you stand a chance. How long has the website been around? Do you have social sharing capabilities embedded in the site? Scott

Keyword tool for news?
You could use Google's last 24 hours option or Blekko's 'search by date' (mainly US-based stuff, I think) and see what gets the most engagement. Google News, Twitter (see which hashtags are popular) and Reddit could be used too, and the 'most read' boxes on various sites like the BBC. It's not quite search interest like Google Trends, but I bet there's a correlation. Trends and other websites (I can't think of others off the top of my head) often have a most searched for section too: http://www.google.co.uk/trends/hottrends

How important are sitemap errors?
Very important. Particularly if you have a large site. We operate a large site with 100,000's of pages and as Dan said it can be difficult to maintain. We use something called Unlimited XML Sitemap Generator which builds XML sitemaps for us automatically. I'd highly recommend it although it takes a bit of fiddling with to get it up and running as it's software which sits on site. We couldn't manage without it as we'd be forever on sitemaps. We found that getting sitemaps right on a large site made a huge difference to the crawl rate that we encountered in GWT and a huge indexation to follow. In particular check for 302's. I made the mistake of leaving those for a while and am sure that we suffered from some loss of link equity along the way. Hope it helps Dawn

Any tips for getting access to the Adwords API?
Michelle - we were using the API quite a bit for SEO keyword research, but eventually had the account cancelled by Google - probably because we had a relatively minor Adwords spend in relation to our API usage. In the meantime we ended up using the SEM Rush api. It's pretty easy to use and they seem to have access to a pretty deep set of keyword data. I know other people, including SEOMoz have had the same issue: http://www.seomoz.org/q/loss-of-google-adwords-api I tried out a few other APIs at the time. Bings tool seemed to have potential, but it only returned very limited keyword data. I think I looked at Wordtracker and another one, but they seemed to be quite expensive compared to other options, based on the pricing model they used.

Best internal linking structure?
Ya i would not over do it. Internal linking is for contextual use, although it plays a slight role in SEO its not the end all be all of SEO. IE is for user benefits to keep them on your page and find the right content. If you have 1 million pages, your goal should not be to rank EVERY PAGE. I would look at Todd Malicoats AI. Duplicate content on the same page--is this an issue?
You may or may not have issues, but the general consensus is that if you place the rel canonical tag to the mobile page you should be ok, perhaps try it on one or two pages and then leave it for a month or so and watch your ranking trends..... Also you could try and ask this question on the G webmaster forum to try and get a reply from a G employee. Best of luck for today and forevermore!

Transitioning to responsive design--should we re-direct m.?
Very well elaborated, maybe I should have done so myself in my original response. Always try 301 to relevant pages in this situation but if the redirect rules prove a headache (and the site has little to no inbound links) to create then I would simply remove the subdomain and 301 to main domain homepage. Somewhat of a Time vs Overall Benefit issue, or in some cases a Cost vs Benefit issue. If the m.domain variant has links pointing to it then it's always beneficial to 301 the links the refering sites are pointing to - I'd try to do this as an absolute minimum in this situation. It is common for mobile versions of sites have no links pointing to them apart from redirects from the main site so in that situation a 301would likely have zero affect whatsoever.
