Google ranks better apps over articles
Nope, it is not an application name. The keyword is broad and generic, but by no means is an application name. I used to rank in the first places with content (a blog post) and now there are no articles ranking in the first 15 places, just web apps and social sites. Let´s say that the keyword is more a "category", very popular in the last year or so. Thanks.

Ads above the fold penalty. Should I request reinclusion?
Yes, I have it all. Not sure about incoming spammy links. I did almost everything to my site (datafeeds, lots of ads, duplicate content, etc.) but never engaged in spammy links. I will try to find some other way to show ads and see what happens. Thanks!

Just noindexed and redirected junk from my site. Now what?
You can also use the "Fetch as google" in Webmaster tool, submit your sitemap for index,, it takes a day to get the results back brom the crawler. The number of submissions are limited to 10/month.

SEO for Image only posts
About the CSS: i have no idea, but i noticed it in other posts ... What browser are you using? If you're afraid of keyword stuffing, you could: remove the title-tag from TEXT-link include both IMG and TEXT in one link with one title=tag include both IMG and TEXT in one link without title=tag, only ALT-tag If the image also serves as a link you could potentially include both ALT and TITEL tag. But with a TEXT-link beneath, perhaps you would be keywordstuffing. SEO wise (both IMG and TEXT) it would be best to use one ALT and one TITLE per image/link combo.
