It's best to send the customer a link to your google maps listing asking them to put the review there themselfs. As you pointed out above if you had 10 reviews all from the same google ID and IP address it will get flagged by google.

You need to add the following into robots txt. The crawler for Moz is called Rogerbot. The below will block the crawler.
User-agent: Rogerbot
Disallow: *
Are they ranking within the top 100 for the term at all?
Could be an Anchor issue around the term, its hard to say as can't see the team or the site.
Also I never really believe everything Moz tells me as they have the same limits as every tool out there, are you checking other tools to see if there is any other issues with the page/site.
Sadly the support for Google maps is pretty bad. Did you report just an issue or did you fill in a form?
If you filled on a form with them than they will call or email you Normally within a week. Sadly no known way to speed it.

You are right Everyone plays this differently. The way we do it is group keywords for one page. No point trying to overdo it as google might see those pages as a doorway page Click here to see more. This normally hits larger sites but i have seen smaller sites get done by this too.
Its better to play itself and put a good 1000 words onto one page with a group of keywords than have 5 pages with less useful content.

This is a hard one, if H1 and the URL are the same and there is other issues with keyword stuffing then maybe look at changing it. I would never change URL's unless I 100% know that's the issue as it could give you more harm than good. If it's not hurting your client now I wouldn't change it. No point fixing something that's not broken in the way of rankings. From a click through point of view it doesn't help having a really long URL.

We took on a client last year with the same thing but they were a franchise business. They had 45 locations which we made into one very large website. I think 3,000 plus pages now days. The key we worked out was to not have any duplicate pages at all from a content point of view.
Each location we run as its own SEO campaign as to get the best outcome from a local SEO point of view.
We are about to move away from having each location on its on wordpress as it's way too much work to keep updated and when we make changes it's hard.

A few things here, if I recall the ones that are showing up highlighted are sometimes ones you have seen or done a search for.
As for your map listing there is lots you can do. The most important things to do is the following:
Do local listings on yelp and other sites.
Insure you have the same address and business name on everything including your website. Little things like Street and St can be seen as different.
Put a google map on your website with the same address.
Ask your customers to do reviews. Be careful don't to do them yourself as Google does check lots of reviews from the same IP address. 
These things can help get your map moving up.
Sadly there really isn't one. I know it doesn't help but the 301 shouldn't have been done in the first place. Changing URL's and doing 301's always impact on rankings in a big way.
You will be best to 301 the new domain back to the old one and try to stay away from doing it. Also keep in mind every time you do a 301 you will only get 70-80% of the benefit of the links going to that domain.

the best way I look at it is if its not hurting you in the way of money or time than I wouldn't worry about it. I have had clients change domains or business names and it can cost a lot of time and money. Where the traffic isn't costing you anything.
If lots of people were contacting you and wasting time than yes maybe look at it but to change everything seems like a lot of work.

I normally stay away from yellowpages as google can see this as spam and even more so when you have 2500 coming from one site. I normally disavow them altogether. If they acted like a other local listing sites and didn't give so many links it's fine but yellowpages is known to do this for sure.
