Multiple links on same page and multiple ref= attributes

The question on StackOverflow seems a tad different the one I'm reading here.
On stack it seems that there is an unknown rel attribute I would assume this would just be ignored.
Here the question is more; this link is flagged as no follow now there is a follow link. I would think search engines have no problem parsing them both, one as nofollow one as follow.

The question on stackoverflow was regarding the second question. One link with two rel attributes, one invalid ("sunshine"), one nofollow. Html5 spec dictates that the second (valid nofollow) attribute must be ignored, thus leaving the link dofollow.
Background for the second question was that I have seen instances where I entered something like My Site in a blog comment and the site just added the rel="nofollow" attribute, withour removing the existing one.
Regarding the first case, it may not be so easy than one would just "think". For example, it is already known that for same links on the same page, only the first anchor text is counted.

I understand your logic with this statement, but I do not believe that you are correct in your assumption that a double no follow will leave the link as a follow. Most likely the erroneous code will leave the link completely ignored by the bots and even cause them not to crawl passed it. Can you correct the link to remove any unnecessary REL properties?
If the second link is ignored, can this behaviour be changed by adding #foo or ?foo to the url?
In this case, I would remove the first link if possible. Again, if this information is no follow, or contains duplicate links it will really cause more problems than solutions. If these links aren't on your site, but in a linking sources site, contact the webmaster and ask them for help correcting these links so that you can receive the full credit for any link juice.
