Moz Local not recognizing my CSV Listing File After Multiple Attempts

I'm trying to get Moz local to recognize my csv file, and its not seeming to do so. I've tried using Google Spreadsheets, OpenOffice Calc, and have even used a Moz Local generator in Google Docs. I've tried this a dozen times now, and none of them seem to work, and leave me with the error --

We did not recognize the file type you uploaded.

Please make sure the file you upload is in CSV (Comma Delimited) format. You may need to re-save or export the spreadsheet as a CSV. Please note that the column headers are required, and must not be changed.
