Setting up All in One SEO plugin

OK, I obviously haven't had my coffee yet. Now I see what you are saying. You are actually talking and looking at two different things.
Are you talking about the URL structure, or the Title tag?
The first image above is for handling the Page Titles.
There should be another section in All in One SEO that handles your URL structure. (I know that this is in Settings under Permalinks).
Find where your URLs settings are and make sure that you don't have a "tag"/%post-title% somewhere.

I had my coffee and now I may need something stronger. This crawl is a mess as now it hit the limit and has thousands if issues. I suppose I should search each issue and post separately but I believe it's the structure set up and having migrated this site years ago. I made the change you suggested, but I did point out something new as the MOZ came up with more info.
We have duplicate content because, as I indicated above the URL
http://www.mydomain.com/page/1000/ is actually a duplicate.
When I place that in the URL box a page comes up and when I click the title the same page comes up with the correct or preferred URL showing the post title not the page. This is replicate through and I found each in the sitemap, therefore, I showing immense errors as "Duplicate Content" so the site is going South fast. I have no idea if this is easily correctable and/or how long it would take to fix the sitemap and resubmit and get it properly indexed.
I manually rebuilt my sitemap, maybe I should delete that in Google Webmaster tools and resubmit the latest. I don't see the page(s) in my example anywhere in my sitemap either.

It looks like the issue may be with some of your widgets. For example the category widget links to /?cat=2, which is also accessible via /category/politics/
So crawlers will see these pages as duplicate content.

I don't see the relationship in the two page addresses, one with the actually post title and a matching one with the page at the end. That's where the duplicate content is showing up in the MOZ reports.
If you click on a category the page titles/urls are what they should be.

That's the issue. The drop down category list uses the /?cat=2 where it should use /category/politics/
You can view the same page via two different urls.

I'm also seeing the duplicate content in the tag issue, I have since taken that out of the All In One SEO "Tag Title Format", per your suggestion. I understand this won't correct immediately in the SERPs.
So back to the category dropdown, is this all related to the setup in All in One SEO?
The Category Title format is set as -> %category_title% | %blog_title%
How would this be corrected or should the widget be dropped?

I would make sure that your Permalinks in your Settings section are the same as your Permalinks in your All in One SEO section.

As far as the categories is the widget that's faulty. I used a couple of other widgets and they supplied the correct URL when a cat. was clicked but as I'm cleaning up "too many links on page" I would still need a dropdown widget for this purpose but don't see one out their yet or I simply haven't located it.
Too many categories to apply to the top nav bar.
UPDATE: It came down to applying noindex features in All In One SEO for Wordpress for categories, tags and tax archives. This will eliminate multiple URLs for the same content.
