Might seem simple, but go to your Google My Business page and make sure that there is a green check mark by your name that says "Verified". If you business is not verified then you will not show in Maps.

RE: 404s still showing in GWT
You definitely need to mark them as fixed in order to remove them. If anything wasn't fixed properly then Google will show them as a 404 again.

RE: SEO Template Recommendations - example provided but would welcome any advice
http://www.eteach.com/justindaviesnovemeber
This template is more visually appealing. You will need to make sure there is content on the page to avoid looking like a catalog. I would make sure that the links aren't hidden in a jQuery, but are fully functional as in site links.

RE: Really odd 404 URL's
is there a header.php? I don't know, maybe better to let the pros handle it lol

RE: Redirects Not Working / Issue with Duplicate Page Titles
Did you set up the 301 redirects in the HTaccess file? If you did, then I would remove both redirects and set them up correctly. A 301 tells Google that a page went from one place to another permanently. Eventually it will stop ranking for a certain page. If you have 2 301 redirects going on right now you are confusing everything. I would remove both and set it up correctly.

RE: Content in Tabs
I have thousands of reviews and product descriptions behind tabs and drop down menus on my site. I have to have them this way because I have 20-30k words on each page. I have to organize it somehow. I use h tags in the tab titles to make sure Google knows that the content is relevant. To this date I have not had an issue with the content being crawled and indexed.

RE: Really odd 404 URL's
Is this WordPress or some other platform? Do you have access to your CSS and HTML files? The template of the site? The ahref is just not done correctly for the contact us page.

RE: Optimizing Product Keywords (that are similar)
I don't think that your key terms need to be extremely varied. You want to optimize for the words people are searching for. If people are searching for brown hooded jacket, that should be your key term.
I would optimize like this:
Page Title: Enigma | Brown Hooded Jacket | London
URL: Domain/Category/Product
Meta Description: This Enigma Brown Hooded Jacket........(about 160 characters)
H1 - Enigma Brown Hooded Jacket
Try using the on page grader here to see if there is anywhere else you can improve the page for this key term.

RE: Subfolder ranks worse than the rest of the site
I think that it could just be that the key terms are extremely competitive. I would advise actually having someone take a look at the site in depth. Anything someone says without actually seeing the site is just speculation and maybes. I'm sorry I can't be more help!

RE: Do these items affect Google ranking or Quality Score?
I agree and I disagree. I know in this case it could be considered "low priority" but in all reality, this is the snippet you want showing when someone searches for your keyword, so it really should be optimized with the rest of the meta data, all at one time. Changing bits and pieces isn't the most efficient way to get it done. It is more likely to see a change in rankings when the on page optimization is all completed correctly at one time.
Use the on page grader tool here to double check each page individually. It will give you a list of the things you need to change to best optimize a page for a particular key term.

RE: Rankings drop - we've added user reviews, are they causing over optimisation on page?
Kate,
I honestly do not believe that there is anything to worry about here. It sounds like there was a decrease in search traffic and that is all that has affected your rankings.
Do you know how to find a cached copy of your page? It will tell you the last time it was crawled. Before you do anything else, see if you can add the date and place of origin to the reviews.
If you had a manual penalty from Penguin 2.0 or 2.1 the 3.0 update should have removed any negative effects if the clean up was done properly. Since this update is rolling out at snails pace, it is possible there was a small change that affected your rankings.
After your disavow and link clean up did you build any new, quality, relevant links? Do you have a social presence and allow people to share your images and pages socially? Can any of your existing links maybe have a no so clean link profile? 3.0 really targeted link neighborhoods, which means you could be guilty by association. Take a look at GWT and see what the links to your site looks like. Use OSE to see if there is anything new you may have over looked.
Your on page optimization looks pretty good. If you haven't refreshed your link profile with new links, or, aren't using social media a lot, you could suffer the same amount of rankings loss. SEO isn't successful unless both the on page and off page op are working together.

RE: New pages need to be crawled & indexed
Does your platform automatically regenerate the sitemap and resubmit it to Google for you? If so, then don't update it again. You can fetch and render but you don't have to. Once the sitemap updates, Google will most likely recrawl the page that has been edited or added.

RE: Different Search Results Depending On Location
Geographical location can make a huge difference. So can the browser and whether or not you are logged into a google account when you are looking. Is this a keyword that is tracked with any rank tracking tool? If so, take that at face value. If you want to see what the SERP is, use IE, clear the cache, log out of everything and browse with and incognito window. That is about as virgin as a SERP can get.

RE: Homepage ranks worse than subpages
I would use YouTube instead. Just because it is another search engine itself and can help increase traffic to your site.

RE: Subfolder ranks worse than the rest of the site
All on the subdomain? Google treats your subdomain as a separate site from your domain. If the penalty was on the subdomain level, that is where you need to focus your efforts. You have to treat them as separate entities.

RE: Subfolder ranks worse than the rest of the site
Did you have a Penguin penalty by chance? You said you disavowed bad links, but if you were penalized was the penalty removed? I think the terms that you are targeting are extremely competitive and you need to do some more off site op to get them ranking well. Run a competitive SERP and see what page on looks like.

No need to apologize, I like a good SEO debate! If I can offer any other suggestions don't hesitate to ask.

RE: Is social networking really worth all the effort?
Let's talk about November:
I had 12 shares of products on my site, three messages on Facebook, 124 likes, 20 comments and 18 shares in November.
My total post reach socially was over 22,000.
I received 87 visits from social networks for a total revenue of $36k.
My domain authority increased 4 points with no back linking. (I don't do a lot of back linking right now)
Even without the revenue and traffic an increase of 4 points in my domain authority was well worth the 6 hours I spent on social media last month, wouldn't you say? I measure the success of my social campaigns first by engagement, then by domain authority, and finally by revenue and traffic.

RE: Subfolder ranks worse than the rest of the site
Without seeing the site I am not sure what else it could be. Are the blogs targeting specific key terms? If so, did you analyze them to see what metrics you need in order to compete with the people on page 1?

RE: Is social networking really worth all the effort?
Most social media links are no follow links, especially Facebook and Twitter. Social links aren't meant to be a back linking strategy. The social signals build trust, and validate whether or not a website is really a business. In 2013 about 7% of ranking factors were related to social signals. I think in 2014 that number didn't really change, but I expect it to in the coming year.
Since no one knows the exact SEO recipe there is speculation to exactly how social plays a part in success. I can tell you from experience, a strong social presence helps your organic rankings. I have seen it work for companies, and I have seen companies without a social presence rank as well. Since these platforms are so interactive, there really isn't a better trust signal than a strong, engaging presence.
