Which one is the third/new one? Your canonical tag looks fine. You are sending a bit of a mixed signal by linking your logo to "/index.html" - I'd strongly recommend changing that to just "/" and directing it to the root URL. Google doesn't always handle mixed signals well, and internal links are strong signals.
