How do I get the Google Adwords Opt-In Form in the Ad

I did a search today and notice that the ad in the sponsored listings had a Google Adwords Opt-In Form in the Ad for a newsletter. I have attached an image here so everyone can see what I mean.

Does anyone know how to do this?

I believe that is still in beat testing you can talk to your account rep to get in on that beta.

Hey Stephanie,

I can confirm that David is correct. If you don't have an account rep, just call AdWords and let them know you're interested in seeing if you can be added to the beta. I believe it's open.

Just so you know, when you sign up for a beta, you don't get the data in AdWords ad extensions section (or at least this is how it was a while back). I would suggest, depending on the requirements for the beta, creating a new form specifically for AdWords and tagging it in your mail client. That way you can at least know how it's working before you dive in.
