This means that the pages listed have the same title tag, usually it would be because the two URLs have the same title tag, but in your case I think it's something else.
For each of the pairs listed in your post, one of the pages redirects to the other. Did you recently make a change to the URL structure of the site? If so, it looks like Google hasn't realized that the old URLs are gone yet.
If you didn't make a change, I would use Google Webmaster Tools "Fetch as Google" functionality to see what happens for each URL. If you're getting redirected for a normal web browser but not Googlebot you should update it so everyone sees the same thing.

Open the first two links you posted above under Acupuncture Infertility in New York City - the /female_fertility and /nyc-fertility-clinic.html pages

That was my first thought, but look at the URL for them, once your browser settles - #1 redirects to #2.

The static page examples are now 404 so I can't see those ones - I didn't notice a redirect before.
But the parameter pages redirect. The parameter page is still being found by Google though and if it continues can easily be fixed in the Search Console settings.
