Hi,

Whilst not really a checklist, have you had a look at the MOZ tutorials? Or you could always have a look through the Google SEO Guide.

I am in the process of doing one myself, but this isn't likely to be ready for a little while yet.

I just went through the exact same process. I couldn't find a super concise one either, but here are a few articles that I used which certainly helped me make a checklist in my head! I wasn't as concerned with a checklist of things like H1, MetaTags, Keywords, Backlinks, 301's. I was more concerned with the "physical" rebuilding that was going to happen since everything on our website was changing.
