Niccord, here are a couple of good resources for you:
- http://alanbleiweiss.com/professional-seo-audits/ (see links for parts 1,2,3, &4 )

Agree with Chris.
Niccord, you could easily take Steve's How to Perform the World's Greatest SEO Audit and turn it into a checklist / rubric for analysis.

Along w/Chris and Eli's suggestion, a sneaky way of doing this is inputting this into Google to get an already made form:
website analysis filetype:doc or website analysis filetype:pdf
Try switching up the kw (such as website checklist, seo website checklits and etc.). Good luck!

Like Kevin said:
If you Google: seo audit filetype:pdf - You'll land on one of QuickSprout's SEO Audits that they conducted for another company. It's actually a great read.

I also reference the Moz guide that @Chris mentions above. That and (less frequently) this one - b-s.ee/149L8Cv. A how-to from stateofdigital.com for mobile websites.
I followed @Eli's suggestion and Googled "seo audit filetype:pdf". It's a great template.
