On all those examples the same anchor text is used. Maybe you have overused the anchor text and it has fallen foul of a penalty?
Keep building but use some naked url links and brand name links to the same page or if you have good relationships with some of those sites ask to edit it to link from the company name not from the keyword rich anchor.
Note: I've only looked at the three samples above and have not looked at the backlinks for the site in question.

what do you think about this, if a site has next to no mozrank or DA, then gets some DA within 6 months do you think google treats this as a new site and gives it a short term boost.
i think this is whats happen here, the boost has gone, and we are drifting

I still think that your anchor linking profile is too heavily weighted to the one term as it's over 60% the one keyword phrase. I don't profess to be an "expert" so it would be good for others to pitch in but a natural looking profile would be 80% brand name (company name) or that's the case if you check a site that hasn't had seo done to it.
My experience of an over optimised anchor penalty is that you get relegated to the bottom of the pack in terms of searches. ie. you appear after all other relevant searches. Often this can be around 10 pages.
