I just ran a grade report for one of my niche websites and I can't seem to fix the following issue:

Well, I've tried really everything (I believe) but this issue is not going away.

If you wanta a page to rank for keywordA, then it is better not have outgoing links on that page with the link text keywordA.
but dont go all un-natural over this, if it is necessary to have that link text then so be it, but if you cann avoid it then do so.

I cant see it either, not on your home page at least, are you sure it is your home page they are talking about.
They may be looking at this

Hmm I just created an On-Page Report Card for that specific keyword and url.

What url? the home page?

it must be talking of the rel=alternate links
