I have tried a lot of things like link exchange, optimization, etc. but now i am so lost about improving my web-ranking. Can you give me a step by step guidance regarding the same?

One unfortunate aspect of SEO is there is a huge amount of conflicting advice. Fortunately, you found SEOmoz and hopefully you can focus on quality sources of SEO information from here, Google, Matt Cutts and a few other reliable sources.

Overall, link exchanges are not a recommended method of improving web ranking. If it was, then sites would simply link to each other and everyone ranks better. It doesn't make sense from a search quality perspective.

Start here: Beginner's Guide to SEO. It is an excellent manual: very short, very clear and accurate.

Asking for step-by-step guidance is not a good idea.  Every site is different.  You need comprehensive knowledge.  Start with the SEOmoz beginners guide, watch the videos, regularly watch whiteboard friday and read the blog.  Ask specific questions here.
