I guess the only real way is to use site explorer and then contact the site directly... We have been spammed bad by competitors and google is penalizing us.. I have contacted the sites and am waiting for response...

I also noticed nobody suggested geositemaps as a possibility... Interesting expert panel here..

Thanks for your answer.I think a combination of both answers put together are the ONLY LIKELY REASON why natural looking serp result happens. I am confused on why it hasn't happened to more authoritative sites? For example green oaks is a stated funded pysch ward with no active SEO. As they have no need for it. What I would like to know is where to put the local data to produce the result right step has gotten. Maybe footer, meta description? Because I have a site that has the location information within the content both in beginning and last paragraph. It just baffles me that the site is not strong on the keywords mentioned..Though ryan did point out some factors which matter it just would surprise me if google weighted them that much over other local sites that are more relevant..

I feel Ryan you have missed some major points about my original post.
1. Natural looking serp
2. They have no keyword in title tag, meta desription or onpage for the word "residential treatment"
3. It is localized to certain geography
4. no anchor text for the word "residential treatment" pointing to the page
What I see you saying is because there are on page keywords for Dallas texas, etc that these would some how make it relevant to show up for residential treatment serp if you are in that ip location? But that doesn't add up because they have no text on their page for residential treatment. and they dont rank at all for these terms outside localized ip.
You can have one without the other. That would mean that if I serp in residential treatment a burger shop could show up that isnt relevant or even a local home builder whom would have the term residential and dallas somewhere onpage...but they dont...

On that list right step would be because it outranks green oaks...We are getting way off topic. What you share is complete opinion and not experience and frankly it makes basic 101 sense...... That because it has on-page key terms relevant to the city which the serp is conducted it would pull a natural organic listing. But I have built many sites specific to cities and states that do not get this type of organic return locally with much more relevant content.
And the first arrow points to right step 2nd arrow pointing to green oaks was for mere example. Since I was talking about right step in the question I proposed this should be the main example.. Also like when people say I always have the policy of person asking question is right....and then make a case to show how wrong they really are...LOL... I need experts answering this question not a battle of who is right about an arrow and where it is pointing..
