We have also seen this happen. I have a few recommendations that I would adhere to especially when managing multiple accounts such as in an agency setting.

Don't keyword stuff in the company name. Use the legitimate name of the company, I have read several articles and posts that mention the fact that Google merges (or tries to) information from other sources such as infoUSA database records.

It 'seems' like when the city name is placed somewhere in the title it frequently causes issue.

Setup a different account for each client! If you do get an account suspended it will only negatively impact that client an the rest will remain okay. This will also help you track down exactly what got the account suspended in the first place. When there are multiple accounts and many updates made to them at once it is hard to tell what change was the one that caused the issue. Hope that helps... The review process can take a while and I have also seen some posts point out that if your "account" is suspended that you almost have to start over from the beginning.
