Both questions refer to the same, all URLs and inner pages, :). At least that's what im interpreting

1./2. You've answered the question yourself. Also, it is possible that you did not implemented the tags correctly in the others versions. Are those tags in every homepage of the four sites?
Remember that hreflangs works in pairs and need to be referential from every site you point.
Just as an example: if you have 2 sites, exampleA.com and exampleB.com, there should be 2 tags in each site: One stating the targetted language/country for the current site and other for the other language/country specific site.
