What factors, other than ad content and landing page, get Facebook ads disapproved?

We are being driven insane by a Facebook campaign where the ads keep getting disapproved. We have edited the ad to the point that it says nothing at all.
The ad is for laser eye surgery, so I am wondering if this type of ad which refers to medical conditions has stronger restrictions. The ad now says 'Find out more about laser eye surgery by [name of doctor]'
There is a photo of the doctor, but there is no text on the photo.
We have targeted the ad at the 21-65 age group.

This makes me think that it is getting disapproved for reasons other than the ad itself, or the landing page? What other factors could play a role in getting the ad disapproved that don't concern the ad or the landing page?

1. What did the ad disapproval email say was the issue?
2. Make sure you read through https://www.facebook.com/policies/ads/ and aren't violating any conditions
3. If you still can't see the issue, I'd submit an appeal form https://www.facebook.com/help/contact/1582364792025146/

Your ad wasn't approved because it doesn't follow our Advertising Policies, which apply to an ad's content, its audience and the destination it links to. We don't allow ads that use profanity, or refer to the viewer's attributes (ex: race, ethnicity, age, sexual orientation, name) or harass viewers.
How to fix: We recommend focusing on your product or service, rather than the audience, and/or remove the profanity from your ad and/or destination it links to.
We have thoroughly read the advertising policies and can't see any reason why it might be blocked.

Not very helpful message from them.
If I had to guess the issue, it would be one of the following:
- https://www.facebook.com/policies/ads/prohibited_content/personal_attributes
- https://www.facebook.com/policies/ads/lead_ads/health_information
Aside from that, the appeal is the way to go.
