Why is Google displaying inside pages for our sites rather than the index pages?

Most likely a devaluation of links, or overoptimization........
When google starts playing funny buggers and showing different pages instead, you can almost bet money on it that one or both of the above is the cause.

Hi Carrie,
Are you sure you're not getting localized search results? Make sure you're logged out of Google, you may also want to try InPrivateBrowsing (Internet Explorer) or PrivateBrowsing (FireFox) to make sure your results are not tainted by localization.
I have attached what the results look like for me with those keywords, Dayton Ohio.
IMO those are fairly strong rankings, and I am also seeing links to the domain root as top listed..

If this is the case, what's the best way to fix it?

Those results are fine, but we are need to be on the first page for "Milwaukee martial arts" and "Milwaukee lasik."
These sites were there previously for these search terms and now they've dropped and the index pages aren't even showing up. Inside pages are showing up instead.

Milwaukee martial arts jkleeblackbelt.com #1 spot. As well as map result.
Milwaukee Lasik supereyes.com # 4 spot.
Both link directly to home page
I can post images if you want but, that is what I'm seeing from my home.

I see what you're saying.
It was my interpretation that the JK Lee listing was just for Google Places and not an actual search listing, but maybe they are one in the same?
As for the Super Eyes site, you're right, it ranks fine for "Milwaukee Lasik," but it's nowhere to be found when you use the term "Lasik Milwaukee."
Should there really be that much of a discrepancy?
